On a system installed decades ago the most common finding is not a failure at all. It is that the lid is buried, there is no effluent filter, and the outlet tee is original. None of those is urgent and all three quietly shorten the life of the bed.
Dealing with them together during one opening is far cheaper than three separate visits: risers to grade, a filter fitted, and the tee replaced while the tank is already empty.
After that the system is serviceable rather than an excavation every time, which is the change that makes a sensible interval actually get kept.
