Some tanks will not empty by suction alone. Years of neglect compact the bottom layer until it behaves more like clay than sludge, and that material has to be broken up and washed off the floor and walls.
Septic tank cleaning lifts the floor pumping leaves
Sludge that has sat for eight or ten years dewaters and packs down. A hose passes straight over it and takes the liquid, which is how a tank can be pumped and still be two-thirds full of solids the following week.
Cleaning breaks that layer up with wash water from the truck and draws it off in stages. The walls come next, because the scum ring welded around the liquid line is what drops back in and reseeds the whole cycle. By the end the floor and the walls are bare, which is also the only moment the tank can be properly inspected, so it pairs naturally with an annual inspection.
If the tank has reached that state, the outlet side usually has too. Expect the effluent filter to be blinded and the outlet tee to be worth replacing while access is open.

A packed floor raises the working liquid level, which pushes the effective outlet closer to the scum layer. Once scum reaches the outlet it leaves the tank, and the only place it can go is the drain field, where it blinds the soil permanently.
That is the whole economic case for cleaning rather than repeat pumping. Replacing a bed is the largest bill in this trade, set out on drain field replacement cost, and it is almost entirely preventable.
After a clean, the interval resets from a real baseline rather than from a guess. The readings feed straight into a pumping schedule built on your own accumulation rate.
For tanks that have gone a long time between visits.
Pumping removes what will lift by suction. Cleaning adds wash water and mechanical work to shift the compacted floor and the scum welded to the walls. On a well-maintained tank there is no difference; on a neglected one there is a great deal.
